# What is a Care Pathway or clinical workflow?

## Care Pathways are custom evidence-based protocols of care that are automated and monitored using the aTouchAway® App and Dashboards.

 

The Care Pathway is an evidence-based approach to creating a patient's care plan that is determined and followed by the multidisciplinary team, including the patient, family members and clinical caregivers.  

The Care Pathway is deployed via the aTouchAway® App, our mobile health app.  The aTouchAway® App can be accessed from any computer, tablet, or smartphone.  The patient and their care team can view a clear snapshot of the care journey at any moment, remotely coordinate the patient's care and transitions, and effectively communicate within the patient's circle of care.

The aTouchAway® Care Pathway maps out each step of the patient's care sequence to clearly communicate what actions needs to happen and when.  Care Pathways are customized for different clinical areas.  The Care Pathway you're working with may differ from that from another clinical area in your facility.
