Care Pathway History

Patient Managers can access Care Pathway History in the aTouchAway® Dashboards and App.

If a patient is or has been enrolled in a Care Pathway, the history, including responses and submitted vital sign readings (if applicable), is available to access in the aTouchAway® solution.

The Care Pathway History can be accessed through the:

aTouchAway® Dashboards

The Care Pathway History is accessible on the Patient Overview page.  To access the Patient Overview, use one of the following methods.  Your access may depend on your organization, your role, or your permissions.

1. Use Health Overview

On the aTouchAway® Dashboards under Live Monitoring, click Health Overview.    

Find the patient for whom you would like to export data.  You can use the search bar or filters to assist in locating a specific patient. Click the patient's name to access the Patient Overview.  

2. Use Creation / Token

On the aTouchAway® Dashboards under Patient Management, click Creation/Token.

Find the patient for whom you would like to export data.  You can use the search bar or filters to assist in locating a specific patient. Click the eye icon to the right of the patient's name to access the Patient Overview.

3. Use Patients

On the aTouchAway® Dashboards under Patient Management, click Patients.

Find the patient for whom you would like to export data.  You can use the search bar or filters to assist in locating a specific patient. Click the 3-dot menu to the right of the patient's name.  Click Patient Overview to access the Patient Overview.

On the Patient Overview page, click the Pathway tab.

On the Pathway tab, click the name of the appropriate Care Pathway.

All responses and submissions associated with that Care Pathway will appear on the right side of the window.  The information will appear in reverse chronological order with the most recent responses / submissions at the top of the list.

If the Pathway Label field on the Care Pathway setup page is completed, the Pathway label will appear above the date and may help to track Care Pathway history, especially if the patient is enrolled in more than one Care Pathway.

aTouchAway® App

On your aTouchAway® App, find the patient under Your Managed Patients.

Click Pathway.

On the Pathway page, click the View button to the right of the specific date.

All responses and submissions associated with that Care Pathway will appear in reverse chronological order with the most recent responses / submissions at the top of the list.

If the Pathway Label field on the Care Pathway setup page is completed, the Pathway label will appear above the date and may help to track Care Pathway history, especially if the patient is enrolled in more than one Care Pathway.
