Care Pathways contain scheduled patient interactions and result in notifications to Patient Managers. These may include colour-coded flags, forms, and other notifications.
All Care Pathways are customized based on your organization's needs and requests. The Care Pathway depicted here may differ from yours, but the same principles will apply for seeing alerts, responding to alerts, and clearing alerts.
Alert Types
Alert Notification Colours
For the following sections, alerts will appear as either red or yellow.
Red alerts are classified as high risk responses and / or vital sign reading submissions.
Yellow alerts are classified as low risk responses and / or vital sign reading submissions.
Notification Banners
aTouchAway® App
On the Patient Manager's aTouchAway® App home screen, notification banners will appear at the top of the screen. They will be colour-coded based on their severity. Each banner will identify the patient, the date and time of the occurrence, their Patient Group (if applicable), and a brief description of the issue.
Click the banner to open a window that contains the details of the alert including a list of all items that caused the alert and the level of alert.
aTouchAway® Dashboards
If Care Pathway responses and / or vital sign reading submissions raised an alert, a red badge will appear on the bell icon in the top right corner of the aTouchAway® Dashboards.
A number in the red badge will show the number of unacknowledged alerts.
Click the bell icon to open the Notifications page.
On the Notifications page, any unacknowledged alerts will be bolded in the list.
Click an alert to open the alert window that contains the details of the alert including a list of all items that cause the alert and the level of alert.
Alert Window
The same alert window will appear on the aTouchAway® App and Dashboards. The Patient Manager(s) can respond to the alert on either interface.
The alert window will contain the details of the responses and / or vital sign reading submissions that raised the alert. Each response and / or vital sign reading in the window will have a red icon (high risk) or yellow icon (low risk).
The window may also contain a list of intervention options. If there is an intervention list, at least one intervention option must be chosen.
At the bottom of the window, the option will be given to add a note and change the patient's status.
Changing the patient's status means to remove the banner and return other notification markers to green (within normal range). Please note, if vital sign readings submissions caused the alert, this does not change the colour behind the person's name on the aTouchAway® App under Your Managed Patients. The colour there will continue to reflect the current vital sign readings risk level until new vital sign readings are submitted that fall within the normal range.
If No is chosen as the response to changing the patient's status, the option will appear to keep the alert pending. If Yes is chosen, the banner will remain on the aTouchAway® App and different spaces that reflect the alert will keep the red / yellow colour denoting the current risk level.
Click Done.
Patient Status Alerts
aTouchAway® App
In the Your Managed Patients list in the aTouchAway® App, a patient's health status will be indicated by a change in the background colour. In this image, the patients' health statuses, based on responses and / or health data, are red and yellow.
Clicking on a patient's name will bring you to the patient profile page in the aTouchAway® App, and the status colour will be further explained.
aTouchAway® Dashboards
On the aTouchAway® Dashboards, a patient's status can be seen on the Health Overview page and the Patient Overview page.
